Customized data-trending reports
We can create customized monthly data trending reports for individual homes or multi-home groups upon request, based on the group’s “top ten” tags. These reports can be used to drive training efforts and to monitor ongoing quality assessment and assurance. We understand the importance of confidentiality. Information is never shared with third parties except on a collective basis for benchmarking or assessing statewide improvement.
Support for MDS coordinators
One key to success in nursing home quality improvement is the accuracy of the Minimum Data Set (MDS). We can provide conference calls or individual telephone consultation for MDS coordinators and other staff on MDS-related topics and regulatory issues.
CNA skill building
The certified nurse aide’s role is critical to quality of care. CNA interventions are essential to meeting national goals and initiatives. Through on-site visits and consultation, our team assists homes to develop a train-the-trainer program to improve CNA proficiency and confidence.
Our on-staff regulatory specialist monitors “top 10” tags and offers assistance in addressing pertinent regulations and deficiencies. This intensive assistance can help homes comply with state and federal regulations.
Culture change program
Our staff works with nursing and assisted living homes to help them build on current efforts to embrace culture change and person-centered care. The goal is to develop tools and resources to help traditional care homes become more resident-focused through strengthened relationships, and to give residents more influence in decisions regarding their care and daily life.
The Advancing Excellence campaign provides support to nursing homes and tools to help enhance the quality of long-term care. The campaign’s website offers free, practical resources that help you achieve your goals and aid in the development of your next quality improvement project. Together, we are working to enhance nursing homes for residents, staff and families.
