The Winston Clinic in Sheridan is one of the 27 Aledade accountable care organization (ACO) partner practices across the country that will be a part of the Million Hearts Initiative’s Cardiovascular Disease Risk Reduction Model (CVDRRM), an innovative program testing a new approach to lowering a patient’s risk of heart attack or stroke. These practices will receive incentive payments and recognition for the work they do to help manage cardiovascular risk with their patients.
The Million Hearts Initiative is a national effort co-led by the Centers for Disease Control and Prevention and the Centers for Medicare & Medicaid Services to prevent 1 million heart attacks and strokes by 2017. ACOs are groups of health care providers, including doctors and hospitals, who work together to coordinate care for Medicare patients in a program that offers incentives for lowering costs while delivering high-quality care. AFMC works in partnership with Aledade to provide technical assistance to Aledade ACO clinics in Arkansas.
