AFMC’s practice transformation team is here to guide you through every step of the process.
Our multidisciplinary team is trained and experienced in all aspects of practice transformation. Whether it’s getting you started from scratch or helping you meet your final goals, we know what it takes to get the job done. Our team has worked with Arkansas clinics for more than 40 years. We live and work in your community, so we are personally invested in your success.
The Practice Transformation team understands that each practice has unique qualities and will individualize the experience to the needs of the practice. We understand that providers and their staff are overwhelmed with the vast number of changes occurring in healthcare and how this creates competing priorities. The Practice Transformation team is available to not only provide individualized coaching, but to also align PCMH with the multiple reporting requirements of your practice.
