Arkansas health care facilities receive Quality Awards

AFMC honors excellence in health care

The Arkansas Foundation for Medical Care (AFMC) recently presented Quality Awards to more than 50 health care facilities in recognition of their commitment to excellence in health care. The awards were presented during AFMC’s 16th Quality Conference, held May 19-20 at the Hot Springs Convention Center.

The facilities earned the awards either through their performance on specific measures of health care or by creating innovative new programs.

“The Quality Award winners don’t just talk about health care quality — they live it every day,” said Dr. Nick Paslidis, AFMC’s chief executive officer.

Nursing Home Quality Awards

Nursing Home Quality Awards were given to nursing homes that improved in areas such as pressure ulcer prevention and treatment, restraint use, chronic pain management and depression management. Facilities were eligible for three types of awards: Best Achievement, Most Improved and Innovator. Results for Best Achievement and Most Improved were based on quality of care data submitted to the Centers for Medicare & Medicaid Services. Innovator Awards were presented to nursing homes that demonstrated a system change that resulted in improved processes and outcomes. The home must have served as a mentor to other facilities in implementing similar changes.

The nursing home winners were:

·    Ash Flat Healthcare and Rehab Center (Ash Flat) — Best Achievement (3rd place), Most Improved (1st place)

·    Arkansas Health Center (Benton) — Innovator

·    Batesville Healthcare (Batesville) — Best Achievement (1st place)

·    Cabot Nursing and Rehabilitation (Cabot) — Innovator

·    Des Arc Nursing and Rehab Center (Des Arc) — Innovator

·    Eaglecrest Nursing and Rehabilitation Center (Ash Flat) — Innovator

·    Fianna Hills Nursing and Rehab (Fort Smith) — Innovator 

·    Green Acres Nursing Home (Paragould) — Innovator

·    Homestead Manor Nursing Home (Stamps) — Innovator

·    Lakewood Nursing and Rehabilitation Center (North Little Rock) — Innovator

·    Little River Nursing and Rehab (Ashdown) — Innovator

·    Lonoke Nursing and Rehabilitation (Lonoke) — Innovator

·    Ouachita Nursing and Rehab Center (Camden) — Innovator

·    Pine Lane Healthcare (Mountain Home) — Innovator

·    Pioneer Nursing and Rehab (Melbourne) — Best Achievement (2nd place)

·    River Valley Health and Rehab (Fort Smith) — Innovator

·    Stone County Nursing and Rehabilitation Center (Mountain View) — Innovator

·    The Meadows Health and Rehab (Corning) — Most Improved (1st place), Innovator

·    Twin Rivers Health and Rehab (Arkadelphia) — Innovator

 

 

Hospital Quality Awards

Hospitals were eligible for four types of Quality Awards: Best Achievement, Most Improved, Validation and Innovator. Results for Best Achievement, Most Improved and Validation were based on quality of care data submitted to the Centers for Medicare & Medicaid Services. Innovator Awards were presented to hospitals that shared innovative and successful strategies with their peers and acted as mentors to other facilities.

·    Baptist Health Hospitals (Little Rock, North Little Rock, Heber Springs, Arkadelphia) — Innovator

·    Baptist Health Medical Center (Arkadelphia) — Best Achievement, Critical Access Hospital (1st place); Most Improved, CAH (1st Place)

·    Forrest City Medical Center (Forrest City) — Best Achievement, PPS (1st place); Validation, PPS (2nd place)

·    Helena Regional Medical Center (Helena-West Helena) — Validation Award, PPS (1st place)

·    Lawrence Memorial Hospital (Walnut Ridge) — Most Improved, CAH (2nd place); Validation, CAH (2nd place); Innovator

·    Medical Center of South Arkansas (El Dorado) — Innovator

·    Ozark Health Medical Center (Clinton) — Validation Award, CAH (1st place); Innovator

·    Saline Memorial Hospital (Benton) — Innovator

·    Sparks Regional Medical Center (Fort Smith) — Most Improved, PPS (2nd place); Innovator

·    St. Anthony’s Medical Center (Morrilton) — Best Achievement, CAH (2nd place); Validation, CAH (2nd place); Innovator

·    St. Bernards Medical Center (Jonesboro) — Innovator

·    St. Joseph’s Mercy Health Center (Hot Springs) — Best Achievement, PPS (2nd place); Most Improved, PPS (1st place); Innovator

·    Summit Medical Center (Van Buren) — Innovator

·    White County Medical Center (Searcy) — Innovator

·    White River Medical Center (Batesville) — Innovator

 

 

Physician Office Quality Awards

Physician offices were eligible for two types of Quality Awards: Quality Improvement and Innovator.

Quality Improvement awards went to physician offices that made use of an EHR or disease registry to produce a report on a minimum of 50 patients monitoring four of AFMC’s outpatient quality indicators; and who, based upon review of data in
this report, identified the need for and launched a system change.

Innovator awards were presented to physician offices that shared innovative and successful strategies with their peers and acted as mentors to other facilities.

Physician Office winners were:

 

·    Clopton Clinic (Jonesboro) — Quality Improvement

·    Community Healthcare Center (Walnut Ridge) — Quality Improvement

·    Corning Family Medical Clinic (Corning) — Quality Improvement

·    Cowherd Family Medical Clinic, P.A. (Heber Springs) — Quality Improvement

·    Delta Center on Aging (West Memphis) — Quality Improvement

·    Pocahontas Family Medical Center (Pocahontas) — Quality Improvement

·    Sparks Clinic (Fort Smith) — Innovator

·    St. Bernards Senior Health Clinic (Jonesboro) — Quality Improvement, Innovator

 

 

Home Health Quality Awards

Home health agencies were eligible for three types of Quality Awards: Best Achievement, Most Improved and Innovator. In the Best Achievement and Most Improved categories, awards were presented for small, medium and large agencies. Results for Best Achievement and Most Improved were based on quality of care data. Innovator Awards were presented to home health agencies that demonstrated a system change that resulted in improved processes and outcomes. The agency must have served as a mentor to other agencies in implementing similar changes.

Home Health winners were:

·    Approve Home Medical Services (Batesville) — Innovator

·    Visiting Nurses Agency of Western Arkansas, Inc. (Clarksville) — Most Improved Award, Small Agency (1st place)

·    Area Agency on Aging of Western Arkansas, Inc. (Waldron) — Best Achievement Award, Small Agency (1st place)

·    Arkansas Methodist Medical Center Home Health Agency (Paragould) — Innovator

·    Baptist Health Home Health Network (Little Rock) — Innovator

·    Baptist Health Home Health Network (Arkadelphia) — Innovator

·    CareNetwork (Rogers) — Best Achievement Award, Medium Agency (2nd place); Innovator

·    Crittenden Regional Hospital Home Health (Marked Tree) — Best Achievement Award, Small Agency (2nd place); Most Improved, Small Agency (2nd place); Innovator

·    Crittenden Regional Hospital Homecare (West Memphis) — Best Achievement Award, Medium Agency (1st place); Most Improved Award, Medium Agency (1st place); Innovator
