AFMC 2008 PAID BENEFITS
HEALTH INSURANCE
AFMC pays 100% of health insurance premiums for our employees, their spouse, and their eligible dependents, including full-time college students to age 25. Coverage is effective the first of the month following the date of hire. Employees are insured through United Healthcare. The plan features a $20 PCP and specialty copay, $500 individual and $1,000 family deductible, prescription drug benefits, $2,000 out-of-pocket maximum, and 100% payment for childhood immunizations.
DENTAL INSURANCE
AFMC pays 100% of dental insurance premiums for our employees, their spouse, and their eligible dependents. Coverage is provided by Delta Dental and is effective the first of the month following date of hire. The plan features a $50 deductible per family member, 100% of diagnostic & preventive services, 80% basic restorative, 50% per schedule for major restorative, and a $1,500 annual maximum. Orthodontic coverage is limited to 50% payment of eligible charges with a $1,000 lifetime maximum.
PENSION PLAN
All employees receive a safe-harbor contribution of 3% of earnings to the company 401K-pension plan, beginning at date of employment. Full-time employees who are at least 21 years of age become eligible for an additional 7% contribution on the first of the month following their one-year anniversary. The safe-harbor contribution is immediately 100% vested (owned by employee) and the additional 7% contribution vests on a five-year schedule. In addition, employees are eligible to make tax-deferred contributions to the plan beginning at date of employment.
LIFE INSURANCE
An amount equal to 1.5 times basic annual earnings. Spouse coverage is $5,000. Child coverage, dependents 14 days to 6 months, is $1000; dependents 6 months to 19 years (25 if full-time student) is $5,000. Coverage is effective the 1st of the next month after the first day of employment.
SHORT-TERM DISABILITY
This benefit covers only AFMC employees and is subject to a 14-day elimination period. Payment is limited to 60% of weekly basic earnings up to a $1500 weekly maximum for up to 24 weeks. Coverage is effective the 1st of the next month after the first day of employment.
LONG-TERM DISABILITY
This benefit covers only AFMC employees and is subject to a 180-day elimination period. Benefits are payable at 60% of semi-monthly basic earnings reduced by other income subject to benefit maximums. Coverage is effective the first of the month following the first day of employment.
PTO (PAID TIME OFF)
Full-time employees receive 16 days of vacation after completion of introductory period. (Graduated after five years of service) Part-time employees receive 8 days of vacation after completion of introductory period.
EIB (EXTENDED ILLNESS BANK)
Sick leave is provided as an insurance policy to protect an employee's wages in case of an emergency. Full-time employees receive 8 days of extended illness after completion of introductory period. Part-time employees receive pro rata of extended illness based on their FTE after completion of introductory period.
TUITION REIMBURSEMENT
AFMC recognizes the importance of education and promotes the professional growth of employees by providing financial assistance to employees who elect to further their education. Full-time employees are eligible to apply for tuition reimbursement after completion of one year of employment with the company. A maximum of $2,500 will be reimbursed per employee per calendar year.
